Nail Psoriasis: Symptoms and Treatments


Nail Psoriasis is a disease that usually affects patients who also have other forms of skin psoriasis. There are only about five percent of the patients that have these types of nail changes that don’t have another form of skin psoriasis. The untreated nail psoriasis can result in embarrassment and social problems.

Nail changes due to psoriasis is not a contagious disease. The cause of this disease is unknown. However, it is considered to be a possible genetic problem that deals with the immune system or that is triggered by environmental factors.

In a case where a patient has nail psoriasis, but no other form so psoriasis, it will be more difficult for the doctor to diagnose the problem since these are associated diseases.



The Symptoms Of Nail Psoriasis

  • Oil drop or salmon patch lesion is a translucent, yellow-red discoloration in the nail bed resembling a drop of oil beneath the nail plate. This patch is the most diagnostic sign of nail psoriasis.
  • Pitting looks as if someone has made small indentations in the nail with a needle.
  • Beau lines are transverse lines in the nails due to intermittent inflammation causing growth arrest lines.
  • Leukonychia is areas of white discoloration within the body of the nail plate.
  • Subungual hyperkeratosis affects the nail bed, just like the silvery layer of plaque psoriasis. Excessive proliferation of the nail bed can lead to onycholysis so that the nailplate falls off.
  • Onycholysis is a white area of the nail plate due to a functional separation of the nail plate from its underlying attachment to the nail bed. It usually starts distally and progresses proximally, causing a painful uplifting of the distal nail plate. Secondary microbial colonization may occur under the nail.
  • Nail plate weakening due to disease of the underlying structures causes this condition.
  • Splinter hemorrhages are longitudinal black lines due to minute foci of capillary hemorrhage between the nail bed and the nail plate.
  • Spotted lunula. Lunula is the white, proximal “new moon” under the nail. Spotted lunula is an erythematous patch of the lunula.


  • Treatments

    If the patient notices any changes in nail discoloration, then that patient should see a doctor for proper diagnosis. If the patient has other forms of psoriasis combined with nail psoriasis, then the regular treatment of psoriasis would be the same. The patient should try to prevent further damage of the nails. Trim the nails so that their changes are less visible, and lessen the chance of contracting an infection under the nails with proper care. It also reduces further irritation of the nail. Soak the nail and try to keep moisturized. Eliminate any addition pressure with shoes. Wear comfortable shoes if toenails are affected. Leave shoes off when at home. Possibly wear a pair of socks instead.

    If the nail has a fungal infection, a physician will be able to prescribe an anti fungal medication. The doctor will be able to do a biopsy of the skin under the nail and be able to advise on what treatment would be beneficial to the patient.

    Remember that the nails must grow out entirely before you see the final results of the treatment. That takes 6-9 months for your largest toe nails, and somewhat less for fingernails. Remember also that both remedies are necessary!
